Inference-Based Cognitive Behavioral Therapy (I-CBT)

What is I-CBT?

While Exposure and Response Prevention (ERP) has been the first-line treatment for OCD, I-CBT is a valuable second-line option. As I-CBT is becoming more known, some are opting for I-CBT instead of doing ERP at all. 

Unlike ERP, which deliberately triggers anxiety to eventually, over time, reduce distress, I-CBT addresses the cognitive patterns that underlie OCD. This helps the client reshape how they interpret and respond to obsessive thoughts without the need to intentionally provoke anxiety.

I-CBT is a specialized approach within Cognitive Behavioral Therapy (CBT) that focuses on understanding and modifying the inferences, or interpretations, we make about events in our lives. These inferences are often unconscious assumptions or conclusions we draw based on our thoughts, past experiences, and emotions.

How Does I-CBT Work?

Two elements of I-CBT are the doubt sequence and “logic” of OCD:

I-CBT Doubt Sequence

The doubt sequence involves:

  • Trigger: A situation or thought sparks doubt (e.g., touching a door handle).
  • Obsessive Doubt: A persistent worry (e.g., “What if I’m contaminated?”).
  • Consequence of the Doubt: The belief that something bad will happen if the doubt isn’t resolved (e.g., “If I don’t wash my hands, I might get sick”).
  • Anxiety: The stress from the unresolved doubt.
  • Compulsion: A behavior (e.g., washing hands) to relieve anxiety, reinforcing the cycle.

I-CBT focuses on challenging the obsessive thoughts and doubts, rather than challenging the compulsion.

I-CBT Logic of OCD

The “logic” of OCD involves distorted thinking, such as:

  • Facts: Things that are objectively true (e.g., “germs exist”).
  • Rules: Unspoken rules, norms, laws, etc. (e.g., “If I touch germs I will get sick or contaminate myself”).
  • Hearsay: Stories or information that you heard in support of the fear (e.g., “I heard someone got sick from touching germs”).
  • Personal Experience: Overgeneralizing past experiences (e.g., “I once got sick, so it will happen again”).
  • Possibility: Overestimating the likelihood of something bad happening (e.g., “It’s possible I’ll get sick, so I need to act”).

I-CBT helps people challenge these distorted thoughts, reducing compulsions and anxiety.
